Healthy newborn size varies with ethnicity, study shows

For newborn babies, taking ethnicity into consideration may help determine how small is too small, according to Canadian researchers.

They found that birth-weight standards based on population averages did not predict which babies born at or near full term were likely to suffer problems associated with being small for their gestational ages.

"The results are not necessarily surprising, but there has been controversy regarding whether customizing growth distributions is necessary," said lead author Gillian Hanley of the School of Population and Public Health at the University of British Columbia in Vancouver. Based on these results, adjusting the standards may be necessary, she told Reuters Health Many factors influence birth weight, including a mother's weight and height, whether she has given birth before and her ethnicity.

Conditions that restrict a baby's growth during pregnancy can also cause a newborn to be smaller than usual. Babies who are not premature, but still small compared to other full-term infants are at higher risk for longer hospital stays, infections and other problems. Hanley and her coauthor Patricia Janssen looked at data on more than 100,000 singleton babies born in Washington state between 2006 and 2008, 93,000 of whom were born at a healthy weight based on general population averages and ethnicity-specific standards. The infants belonged to white, Chinese or South Asian ethnic groups.

The researchers used the babies' weights to try to predict complications after birth, including an Apgar score - which measures heart rate, breathing, muscle tone, reflexes and color - of less than 7 out of 10 at five minutes after birth. They also examined admissions to the neonatal intensive care unit, the need for ventilation, extra long hospital stays, hypothermia, low blood sugar and infection, all of which indicate newborn distress.

Newborns considered small at birth, meaning they were in the lowest 10 percent of birth weights for the whole group, tended to have the most problems in their first days, the team reports in the American Journal of Obstetrics and Gynecology.

But based on size alone, the researchers were only able to predict a baby's risk of hypothermia and needing to stay in the intensive care unit after birth, whereas considering size in combination with ethnicity predicted all of the potential problems.

"It's now well known that ethnicity plays an important role in fetal growth and that weight references must be developed and adapted to local populations," said Cyril Ferdynus, a biostatistician in Reunion, France, who was not involved with the study but has researched this question.

Special ethnicity-specific birth weight charts are already common in hospitals in some countries. "In UK, over half of hospitals have already introduced customized charts, and more and more are coming on board," said Jason Gardosi, director of the Perinatal Institute in Birmingham, United Kingdom. But in the U.S., the specialized charts are only used in research and not yet in clinical care, which is largely because clinicians are slow to make changes to practice, Gardosi told Reuters Health by email.

Other variables should be combined with ethnicity, he said, since mothers within specific ethnic groups also come in different shapes and sizes. Based on these results, if more U.S. hospitals incorporated ethnicity-specific ranges into charts, they could more accurately identify the infants that need to be watched more closely for complications, Hanley said. "By more accurately identifying the babies at risk of these complications, hospitals could save resources (by not investigating small but healthy infants) and save a lot of parents the anxiety of being told that their small but healthy baby is small for gestational age," she said.

Two men accused of sexually abusing 15-year-old girl at California pot farm

SAN FRANCISCO – Two men are accused of forcing a 15-year-old girl to work on a Northern California marijuana grow, sexually abusing her and locking her in a metal toolbox for days, authorities said.

Ryan Balletto, 30, and Patrick Pearmain, 24, both allegedly had sex with the girl who was cutting marijuana buds for them at Balletto's Lake County property, according to a federal complaint unsealed Monday. The girl, described as a runaway from the Los Angeles area, told investigators she was locked in a 4 by 2 by 2-foot box. Holes had been drilled in the box and water was run through the box to wash her without letting her out, the complaint said.

"Some in our community believe that marijuana grow operations are run by compassionate caregivers interested only in supplying medicine to the sick," said U.S. Attorney Melinda Haag in a statement Monday. "Unfortunately, this case illustrates what we in law enforcement see — marijuana grow operations that include heavily armed, violent individuals, motivated by profit, carrying out abuses of vulnerable victims." The girl came to Lake County Sheriff's deputies' attention when the Los Angeles Police Department advised that Balletto may have kidnapped her.

After deputies visited Balletto's home asking about the missing girl on April 29, the complaint said, she called authorities the next day to say she was OK and in Sacramento. LAPD investigators traced the call and police located the girl at a West Sacramento hotel, where authorities arrested Pearmain, the complaint said. Balletto was arrested when authorities raided his property on May 1. Court paper said officers found sex toys, a rope tied into a noose and a homemade device used "presumably to attach a human and immobilize them" in a trailer Balletto was living in.

Hundreds of marijuana plants and several firearms were found as part of the investigation. Balletto and Pearmain were both charged with marijuana possession and using a minor in drug operations. Balletto is also charged with possession of a firearm to further a drug trafficking crime. If convicted, they face a maximum penalty of life in prison. It wasn't immediately clear whether Balletto and Pearmain have retained attorneys. A call to the Lake County District Attorney's office to see whether they face charges related to the sexual abuse allegations was not immediately returned. The U.S. Attorney's office said in a statement that Balletto and Pearmain have already had their first court appearance. A call to the office seeking further details on the status of their case was not immediately returned.

How to protect yourself from deadly brain-eating amoeba

One of this summer’s biggest health stories reads like something out of a science fiction novel: A brain-eating amoeba that resides in lakes and ponds, entering people’s bodies through their nostrils and ultimately killing them. News that a 12-year-old girl from Arkansas contracted a brain-eating parasite during a trip to a water park has led to widespread concern about the safety of swimming or water sports during the summer.

The truth is, this concern isn’t entirely unwarranted. While I’m not trying to promote mass hysteria here, I do urge people to exercise a bit of caution before diving into any body of freshwater. High temperatures have gone a long way in both heating up and shrinking the size of lakes and ponds. This kind of environment, hot, shallow, stagnant water, is perfect for the single-celled amoeba to reproduce and thrive. While the amoebae do not affect everyone who enters the water, for the few that are sensitive to the organisms, the consequences are nearly always fatal. The amoeba gets up the nose, burrows up into the skull and destroys brain tissue.

Over the next few days, the infected person exhibits signs and symptoms of meningitis, such as fever, headache and stiff neck, before falling into a coma and eventually dying. This condition is rare, and there is no need for mass alarm. There have only been 128 reported cases in the United States since the amoeba was identified in 1960. However, given the fatality rate of this condition – only one person has ever survived – I don’t think it hurts to exercise some caution. While the best prevention is obviously to avoid swimming in any body of freshwater to begin with, I realize this is an unrealistic expectation. I do recommend avoiding shallow water that has been thoroughly heated by the sun, all the way down to the bottom. Amoebae are less likely to be found in deeper water where you can feel that the temperature at the bottom is cooler than the top. Also, make sure to plug your nose when jumping or diving into water and avoid excess splashing. These simple measures can all decrease the likelihood of amoebae entering the body.
